Ordering Mailing Lists (LPC, LPCC & Approved Supervisors)

If you wish to have a mailing list of LPCs, LPCCs or Approved Supervisors for professional counselors emailed to you in an Excel spreadsheet you may request this information by written request to the Board: please specify which list(s) you are requesting, provide your name and phone number, and the email address to which the list(s) can be sent.  The Board charges a fee for each list (see "Copies of Public Government Data") based on the length of each document.  Current fees for each list is indicated below.  Please make your payment by check or money order:

 

     Licensed Professional Counselors:                            $9.50 (as of 8/6/10, 38 pages)

     Licensed Professional Clinical Counselors:                $3.00 (as of 8/6/10, 12 page)

     Public List of Approved Supervisors:                        $8.00 (as of 8/6/10, 32pages)

 

The mailing lists for LPCs and LPCCs include: each licensee's name; license number; license status; designated mailing address and phone number; issue date; and expiration date.  The mailing list of Approved Supervisors (with a public designation) includes: each supervisor's name; date approved; and designated mailing address and phone number.

 

To order a mailing list of Licensed Alcohol and Drug Counselors (LADCs), please use the LADC mailing list request form.  (If you choose to order lists from both the LPC/LPCC program and the LADC program, please provide separate payments.)
